Passport Requirements: 
Please submit your valid and signed passport with a minimum validity of greater than six months and one blank visa page. If additional pages or if your passport needs to be renewed, please follow this link:

Photograph Requirements: 
Two most recent passport-style photograph (2x2) with light background. Homemade photos are not acceptable.
Documents Required: 
-A copy of airline flight itinerary or letter from travel agent with applicant's full name.

-A copy of valid Driver's License. (If a copy of driver license not available, a copy of the utility bill such as water, or electric bill may be used with applicant's full name and address)

-For applicants who resides in the following state Driver's license copy also required to be NOTARIZED (Alabama, Arkansas, Colorado, Kansas, Louisiana, New Mexico, Mississippi, Oklahoma, South Carolina,Texas)

-For applicants who resides in the following state also require to provide NOTORIZED copy of most recent bank statement (Alabama, Georgia, Mississippi, South Carolina, Tennessee)

-For applicant who resides in the following state require a signed by the applicant with detail of the trip (Florida,New Hampshire, Massachusetts, U.S. Virgin Island, Puerto Rico)
