Passport Requirements:
Please submit your valid and signed passport with a minimum validity of greater than six months and a blank visa page. If additional pages are needed or if your passport needs to be renewed, please follow this link:
Photograph Requirements:
Two most recent passport-style photograph (2x2) with light background. Homemade photos are not acceptable.
Documents Required:
- Business Letter - A letter from the company within the US on the company letterhead is required. The applicant must include employment status within the company and clearly define the purpose of travel. Financial guarantors must be provided as well as a person of contact and their contact details.
- Flight Itinerary - Please submit a copy of your flight itinerary and/or a copy of your airline tickets.
