Photograph Requirements:
Two most recent passport-style photograph (2x2) with light background. Homemade photos are not acceptable.
Documents Required:
- Flight Itinerary - Please submit a copy of your flight itinerary and/or a copy of your airline tickets.
- Minors - If applicant is under 18 years of age, please complete an additional application form signed by a parent. A copy of the child's birth certificate and copies of the parents' passports is also requested. If traveling with only one parent, a letter of consent is required from the child's other parent, allowing the child to attend the trip.
